That is why we are offering this service to you. We will walk through your medical complaint and ensure your time off was appropriate and develop a plan to get your returned to work!*

*ZYP Medical, LLC will not provide work excuses for greater than 5 consecutive dates. We also cannot provide evidence or support in the form of a work excuse for missed days >3-5 days prior to your visit. ZYP Medical, LLC does not provide work accommodations, work restrictions, short term disability evaluations or letters, long term disability evaluations or letters, workers compensation evaluations, or any other service that would require follow up or legal constraints tied to your employment. Also keep in mind, ZYP Medical, LLC is not responsible or liable for your employment status and cannot guarantee your employer will accept the letter provided. Depending on your state laws, employers may reserve the right and discretion to reject medical work excuse letters created by medical professionals.

Work Note Policy

At ZYP, we strive to provide efficient and effective medical care while adhering to the following policies regarding work notes:

  1. Duration of Leave:

    • We can provide work notes for up to 5 consecutive days off. There are no exceptions to this. You (the patient) are responsible for follow up care and any work notes that require additional time off past the recommended time off given to you by your provider at ZYP Medical, LLC. 

  2. Timeliness of Excusals:

    • Work notes can only excuse missed work that occurred within a reasonable time frame, typically within the past month. We cannot retroactively excuse absences that occurred outside of this window.

  3. Work Accommodations or Restrictions:

    • We do not provide work accommodations or restrictions. For specific medical needs affecting work or an injury that occurred while at work, we recommend talking to your employer about opening a worker’s compensation case or seeing an occupational health provider.

  4. Workers Compensation:

    • We do not offer evaluations, services, or notes related to workers compensation claims. Please consult with a qualified provider specializing in occupational health for such needs.

  5. Additional Notes:

    • All work notes provided are based on the patient's current medical condition and are subject to medical evaluation during the visit.

    • We cannot extend work notes past the recommended time frame given by your ZYP Medical provider. 

    • ZYP Medical LLC or any of its associated medical providers are not responsible for the status of your job, duties, or any other work related matters. 

    • ZYP Medical LLC cannot guarantee that your employer will approve your work excuse letter. ZYP Medical will not legally represent you in any work related matters if you (the patient) were to file any legal claims against your employer.

    • ZYP medical LLC will not complete any FMLA (Family and Medical Leave), Short term Disability, or Long term disability paperwork. This must be done with your primary care provider. 

    • ZYP Medical LLC and its associated medical providers are NOT your primary care provider.
